"Dickhead" (kukskalle) is used here, and has developed independently of the English version, and it is a much much more serious insult here, so I would consider it a different insult.
These days most of our insults seem to be anglicisms; either translated (mora di! as in "yo mama!") or simply used in the original English.
Which I find kinda sad...
The Swedes, on the other hand, have lots of interesting insults, like "Shit in your hand!", "Throw yourself at the wall!", "Bite your thumb!" or "Pull something old over yourself!"

I don't know if you can call this regional, but it does come from an American genius of comedy:
'May the fleas of a thousand camels infest your armpits.'
As for standard insults, we're pretty lame, things like 'Ass!' 'What a prick,' Dickhhead,' as sander said, 'Dick,' 'Cocksucker,' 'Bastard,' 'Got your head up your ass,' 'Got a stick up your ass,' 'Stick it in your ear,' 'Stick it where the sun don't shine.'
Now you will typically hear more colorful, creative insults, but they're generally made up on the spot.
