I've tested your map myself. I found a few icons not showing there "disabled" form. I'm also map maker but not well known yet... Did you import the partner of each custom icon with a label, "DISBTN"? I also experienced it myself once. Just use the ff. paths for the specific type of icons:
BTN: "ReplaceableTextures\CommandButtons\(NameofFile)"
DISBTN: "ReplaceableTextures\CommandButtonsDisabled\(NameofFile)"
After this... Save your map, load it again and POOF! It works! Anyway, your map is GREAT! I give it all thumbs raised! 8)
